How can we recreate it: Change the block

That's not MCP's fault, that will happen on any server when you mine too fast. I recommend using Efficiency 3 or 4 as opposed to 5 on your pickaxe to fix the problem.

It's a glitch block. It's on mojangs side if you place a block where the glitch is then it'll go away.
