How Long Have You Played On MCP? I have been on for about 7 hours, I believe.
Have You Ever Been: Banned, Muted, Kicked? I have been banned from a server when Minecraft first came out.
If You Have Been, Why, And For How Long? I was banned because I did not know better when I was of that age, and someone had hacked and ranked me up. I thought it was the owner, so I switched my game-mode to creative, then the actual owner had logged on and banned me. (That was on my previous account, staceylove123.)
Why Should We Pick You To Be Our Next Moderator? Although I haven't been a part of the community for that long, I strongly believe that I should be picked as Moderator because I could bring a lot of good things to the community, I love to help people, and I could dedicate a lot more of my time to the server.

you didnt put your username lol , you might want to get some more playtime for a chance at mod also the Have You Ever Been: Banned, Muted, Kicked? question means only for this server but  if thats the only time you have ever done something bad thats good i guess
