Ban Management error

What is the bug: Ban Management advanced search fails 100% of the time.
Where is this bug located: Ban manager by FrostCast http://cdgs.net/MCBans/index.php?action=...player=%25
How can we recreate it: Click 'display all' on the main ban manager page http://cdgs.net/MCBans/

It's something to do with you, works fine on chrome, firefox and ie/edge
